Job Description
The IHS Coordinator assists individuals in developing/increasing independence and enhance their level of self-sufficiency through training and coaching. Primary duties include: Provide information, coaching and support to assist individuals in achieving their goals in all areas of independent living (housing, banking, shopping, budgeting, bill-paying, grocery
shopping, community integration, etc.) Assists individuals in identifying resources within the community that fulfill their particular needs. The ideal candidate should be self-motivated, comfortable working independently, have strong interpersonal skills and the ability to
communicate effectively and professionally.
***Company Vehicle Provided for Community-Based Work***
Requirements
*High School Diploma or general education degree (GED) required; Bachelor’s degree (B.A.) in Human Services related field; or two years experience working with people with disabilities preferred. Successfully complete DHS background study prior to beginning employment.
*CPR and 1st Aid training provided once hired.

Job Description
You will provide on-going support to participants enrolled in enrichment programming, life skills classes and/or at the work place. These supports are provided at either ProAct or in the community. Reporting and fact-to-face counseling are also responsibilities of the Direct Service Professional.
Supports individuals with disabilities while interacting in the community. Plans and prepares class curriculum/lesson plans to fit individual needs and teaches classes as required. Provides occasional job coaching. Assists with behavior management plans. Completes all record keeping in a timely and accurate manner, including but not limited to consumer case notes, behavior observation charts, seizure logs, daily logbook, and accident/injury reports. Assists with consumer’s personal care needs including but not limited to toileting, feeding, food preparation/set-up, break room supervision, dressing and washing. Provides daily coverage for lunch. Uses a company vehicle to transport individuals.
Requirements
